Medications and Other Medical Needs

Hello Spangler Families!
Does your child require any type of medication to be administered at school, such as hydrocortisone cream, an EpiPen, Tylenol, or another medication?
If so, please read the following important information regarding medication administration at school.
In order for Mrs. Madison, our Health Clerk, to properly and safely care for your student, we must have your child’s care plan on file, along with the appropriate medication authorization form.
Please review the three attached forms:
- Self-Carry/Administration Form – For students who are able to safely carry and administer their own medication at school.
- Medication Administration Form – For students who require an adult on-site to assist with or administer their medication.
- Anaphylaxis/Allergy Form – For students with allergies that may result in an anaphylactic reaction.
After selecting the appropriate form, please forward it to your child’s physician for completion. Once the physician has completed the form, you may return it to the school through ParentSquare, email, or in person at the school office.
